Why Health Is a Dynamic Biological Process

What Do We Mean When We Say Someone Is Healthy?
Ask ten people what it means to be healthy and most will describe the absence of illness. They may tell you they have normal blood tests, take no medication or have never received a serious diagnosis. Implicit in each answer is the same assumption: health is a condition we either possess or lose.

It is an assumption that has shaped medicine for generations. Yet it raises an important question. Have we become so successful at identifying disease that we have overlooked the biology that creates health in the first place?
Modern medicine has understandably reinforced this perspective. Over the past century, extraordinary advances in diagnostics have enabled physicians to identify disease with remarkable precision. Blood tests detect diabetes years before complications develop.
Medical imaging reveals tumours that were once invisible. Genetic testing identifies inherited risks that previous generations could never have recognised. These achievements have transformed medicine and saved countless lives.
Yet they also reveal an interesting paradox. While we have become increasingly sophisticated at diagnosing disease, we remain surprisingly limited in our ability to define or measure health itself. Normal laboratory results certainly reduce the likelihood of disease, but they do not necessarily tell us how effectively the body’s physiological systems are functioning, repairing or adapting. This distinction lies at the heart of functional medicine, which seeks to understand how biological systems work together long before disease becomes clinically apparent.
This distinction may seem subtle, but it fundamentally changes how we think about health.
Health Is an Active Biological Process
One of the defining characteristics of living systems is that they are never static. At every moment, billions of cells are repairing DNA, replacing damaged proteins, producing cellular energy, regulating inflammation, balancing hormones and responding to changes in the external environment. Even during sleep, the body is actively repairing tissues, consolidating memories and restoring the physiological systems that make tomorrow possible.
These processes continue regardless of whether we are consciously aware of them. They represent the continuous work of maintaining life, preserving stability and responding to change. From this perspective, health is not best understood as a destination that we eventually reach, but as an ongoing biological process that the body performs every second of every day.
This systems-based view of health reflects one of the central ideas in systems biology. Rather than viewing organs as isolated structures, systems biology recognises that metabolism, immunity, the nervous system, hormonal signalling and cellular energy production function as an interconnected network. Health emerges not from the performance of a single organ, but from the coordinated interaction of the entire biological system.
The absence of disease is therefore not the definition of health. It is often the consequence of these adaptive processes functioning as they should. Viewing health as an active process rather than a fixed state inevitably changes another important question: when does chronic disease actually begin?
The Years That Medicine Rarely Sees
This perspective also changes how we understand chronic disease. Most long-term illnesses do not appear suddenly. Instead, they emerge after years of gradual physiological change, during which the body continues to compensate despite slowly declining function.
Sleep may become less restorative. Recovery after exercise takes longer. Mental clarity becomes less consistent. Energy fluctuates. Resilience to psychological stress gradually diminishes. Individually, these changes are easy to dismiss as part of modern life or simply getting older. Collectively, however, they may represent the earliest signs that the biological processes responsible for maintaining health are becoming less efficient.

These early physiological changes are particularly relevant to healthy ageing. One of the goals of preventive medicine should not simply be to diagnose chronic disease earlier, but to recognise declining biological function while there is still an opportunity to preserve resilience and extend long-term health.
By the time conventional investigations become abnormal, these changes have often been developing for years. Disease may seem to appear suddenly, but the biology that preceded it rarely does.
A Clinical Perspective
Whether I was working as a surgeon, coordinating humanitarian health responses with the World Health Organization, or later practising functional medicine, one observation kept resurfacing. Patients rarely became unwell because a single organ or physiological system suddenly failed in isolation. More commonly, several interconnected systems began functioning less efficiently long before a diagnosis could explain what they were experiencing.
Energy production gradually declined. Recovery slowed. Sleep became fragmented. Low-grade inflammation persisted. Muscle mass decreased. Metabolic flexibility narrowed. Individually, each change appeared modest. Collectively, they reduced the body’s capacity to adapt to the ordinary demands of life.

This observation has gradually changed the questions I ask in clinical practice. Rather than focusing exclusively on identifying disease, I have become increasingly interested in understanding which biological processes are beginning to lose efficiency, often long before pathology becomes obvious.

The Tao Perspective
Perhaps one of the greatest opportunities in medicine is not simply to diagnose disease earlier, but to understand health more deeply.
If health is viewed only as the absence of measurable disease, then medical care inevitably begins when physiology has already started to fail. If, however, health is understood as the body’s continuous ability to adapt, repair and recover, then preserving these processes becomes just as important as treating illness itself.
This perspective also explains why I have become increasingly interested in root-cause medicine. Rather than viewing symptoms as isolated problems to suppress, the more important question often becomes whether the biological systems responsible for creating health are continuing to function as they should.
This idea forms the foundation of the Tao Framework. It invites us to shift our attention from asking “What disease is present?” to asking “How well is the biology that creates health still functioning?” That question does not replace modern medicine; rather, it broadens our understanding of what it truly means to be healthy. Perhaps the future of medicine will not depend solely on finding disease earlier. It may also depend on learning to recognise, preserve and strengthen the biological processes that create health before disease ever becomes inevitable.
That is where the Tao Framework begins.
